Re-Opening Safety Compliance Waiver

 As we re-open the church for physical gatherings, this waiver serves to assist the Bethel LA Church (herein referred to as the Church) in protecting the health and safety of attendees of physical gatherings from the potential threat of the Covid 19 virus.  

The Church is re-opening and operating under the health and safety recommendations of the Los Angeles County Public Health Officer, the  Center for Disease Control, and the State of California. By attending physical gatherings at Bethel LA you understand that you are entering a  building that is open to the public and that you enter at your own risk. The  Church is not liable and is to be held harmless and blameless should you contract the Covid 19 Virus while attending a physical gathering at the  Church.

By signing this waiver, you agree to abide by the safety protocols and directions provided by the Church including but not limited to; wearing face coverings, temperature checking, hand hygiene, and observing social distancing. Further, you agree that this waiver may be used for tracing purposes to contact you should an incident of the Covid virus be reported at the Church during the time of your attendance.
